Introduction:

The Assessment and Treatment centre at Wythenshawe hospital provides a multidisciplinary team approach to falls treatment. The day hospital provides a falls management programme that is individually tailored to the patient. It includes confidence building, group work, Tai Chi, home exercise training programmes etc.
During their time here the patient will have access to a multidisciplinary team that includes Physiotherapy, Dietitians, a Consultant Geriatrician, Podiatry, Pharmacy, Orthotics.
A specialist falls clinic is also held on a Thursday during which the patient may be assessed by the Consultant Geriatrician and their multidisciplinary team.

What are the referral criteria?

There are only a few criteria for referral to this service. All referrals will be triaged by the extended primary care service. Those appropriate for this service at Wythenshawe will be passed directly to the team. This triage is dependent on the referral information, and so it is very important that the information provided is accurate and up to date.

Who can refer to the service?

Anyone can refer into the South Manchester falls service, however the person making the referral must be in contact with the patient’s GP and the referral must be made via the GP. By referring via the GP, the referral can be done electronically and a full history can be obtained, therefore the patient will be seen by the most appropriate member or the team.
