Introduction:

The community falls service comprises three nursing sisters and two staff nurses experienced in care of the elderly and in assessing the needs of people who have fallen.

The community falls service teams provides a service to anyone over the age of 18 who has had a fall or is considered at risk of falling and is either registered with a central Manchester GP or lives in the central Manchester area.

What does the Community Falls Service Provide?

Within 5 working days of receiving a referral the CFS will assess the information provided on the referral, and if appropriate, contact the patient to arrange a mutually convenient time to visit. During the initial visit a specialist assessment is undertaken, designed to identify risk factors associated with falling.
CFS will discuss with the patient and/or carer the interventions that are required to minimise the risk of a fall and will agree a suitable action plan, which may include a series of referrals to partner organisations being generated.
Before leaving, CFS will provide the patient /or carer with written information and a visiting card explaining how to contact the service.
Most patients remain under the care of the CFS team for a maximum period of 3 months.
