Introduction:

The extended primary care falls services are based at the new community hospital in Withington. Here the patient is offered a full assessment from a physiotherapist and podiatrist. In addition, a physiotherapist now offers home visits for housebound and frail patients. The patient is assessed for falls risks, and they are signposted to the most appropriate healthcare profession or community team including pharmacy or medical assessment.

What are the referral criteria?

The person must be resident in south Manchester or be registered with a south Manchester GP.

Who can refer into this service?

All referrals must be made via the patients GP. Anyone can refer with consent from the GP including nurses, active case managers, allied health professionals.
