Introduction:
The aim of PACE is to promote physical activity within the health wards of North and East Manchester. It focuses on offering community based activities that will help individuals to carry out daily routines such as gardening, housework and walking.
They provide readily accessible venues such as community centres and church halls, social clubs and allotments, thus helping to provide a wide range of activities.
What are the referral criteria?
The project is for anyone over 18 years old with long term health conditions or with risk factors for developing heart disease in the future.
What does PACE involve?
- 30 minute consultation: an assessment of lifestyle and physical activity levels. During this consultation the client may become a PACE member if they wish.
- Follow up consultations: these takes place at 12 weeks, 6 months and 12 month intervals.
- Blood pressure, resting heart rate, height, weight, body mass index and body fat percentage are worked out at each consultation.
- PACE delivers a variety of physical activity opportunities such as: swimming, gentle exercise classes, fitness centre workouts, badminton, local walking groups, cycling, home based exercise, Tai Chi and many others.
- PACE can also refer its members to NHS services such as cardiac rehabilitation Phase III, weight management services, expert patient programme and falls prevention.
Who can refer to PACE?
Referrals can come from the following people:
- Self referral
- General Practitioners
- Physiotherapists
- Health visitors/ Cardiac nurses/ Practice nurses
- Other health professionals.
