Introduction:
The SMILE project is a service provided by Manchester PCT in partnership with South Manchester Healthy Living Network and a range of organisations providing physical activity opportunities.
The aim of the scheme is to improve the lifestyle of the individual by increasing long-term participation in physical activity. The scheme aims to improve awareness of the benefits of exercise and that it can help with the management of CHD, diabetes, and obesity.
Who can refer into the scheme?
Referrals can be accepted from a wide range of sources including GP’s, practice nurses, specialist nurses and physiotherapists, within the South Manchester area.
What are the referral criteria?
The person must be resident in the Manchester PCT area and be registered with a South Manchester PCT GP. The client must be over 16 years of age and be clinically stable. They should also be currently inactive.
Before referring a client please check that one of the criteria below is satisfied:
- CHD risk factors including hyperlipidaemia, controlled hypertension (current BP 180mmHg systolic and 100 mmHg diastolic or below), controlled diabetes, mild to moderate obesity (class I and class II)
- Stable angina
- History of MI, CABG or angioplasty and have attended phase III cardiac rehabilitation.
The following conditions are currently excluded from the programme:
- Blood pressure over 180 mmHg systolic and 100 mmHg diastolic.
- Unstable angina, cardiac patients who have not completed a phase III exercise programme.
- Uncontrolled asthma/ respiratory diseases.
- Unstable psychiatric disorders.
- Children under 16 years old.
How to refer into the scheme.
Referrals can be accepted from health professionals within the South Manchester area. The health professional should fill in the exercise referral form and give as much relevant information as possible including medication and medical history. A current blood pressure and heart rate reading should also be included. It is important that the form is clearly filled out and signed by the health professional.
The application forms are in triplicate and can be ordered from the SMILE project directly using the form in appendix j. One copy must be sent to SMILE, one copy kept at the surgery with the patients medical records and the third copy given to the patient to take with them to their first appointment at the scheme.
